Coming home after a rehab stay shouldn’t feel overwhelming, but it often does. Patients and families may face unexpected physical challenges, complex discharge instructions and new caregiving duties. Our Go-Home Packages are designed to ease this transition from rehab to home. With support from professional caregivers and certified nursing assistants (CNAs), we help with post-rehab support, personal care, meal prep, transportation, and understanding discharge instructions. Rhythms Home Care gives you the helping hands you need right when you need them.

Caregiver Packages

4-Hour Package

$180 ($45/hour plus mileage for caregiver's vehicle)

  • Transportation home
  • Personal Care (shower, bathroom assist, dressing assist)
  • Grocery shopping
  • Meal prep
  • Laundry and basic housekeeping

Companion Care | 6-Hour Package

$270 ($45/hour plus mileage for caregiver’s vehicle)

  • Includes all services from the 4-hour package, with additional two hours of care

Certified Nursing Assistant (CNA) Packages

These packages are best suited for clients needing transfer assistance or returning home with a catheter.

Initial Nurse Assessment

$200 (Waived if additional services are added beyond 60 days)

4-Hour CNA Package

$200 ($50/hour plus mileage for caregiver’s vehicle)

6-Hour CNA Package

$300 ($50/hour plus mileage for caregiver’s vehicle)

Returning home after a hospital stay or rehabilitation can be overwhelming, especially during the first few weeks. You’re managing new medications, unfamiliar treatments, and changed routines while recovering.

A transitional care program provides essential support during this vulnerable period. With a trained companion as part of your care team, you’ll have someone to help navigate medication schedules, coordinate appointments, recognize warning signs, and adjust to new routines. This support can prove beneficial and reduce your chances of needing to be readmitted to the hospital.

For a personal care assistant client, 24-hour notice is sufficient for us to sign the service agreement and get the care scheduled. For certified nursing assistant care, we would need 72 hours notice, mainly because there is always a delay in getting orders for doctors offices. If we are doing a rehab to home or hospital to home visit, this shouldn’t be an issue as we should get the discharge paperwork pretty quickly.

No, if your doctor has ordered any medical care our services are simply an addition to those services. Often, therapy and nursing services are limited in scope under home health due to Medicare and insurance requirements. Our transitional care services are tailored to your additional needs and allows us to give you that extra support you need to make a smooth transition to home.

No, this program is one that would not be covered by insurance. Medicare and most insurance plans generally cover very specific skilled services like therapy. While those services are essential, they don’t provide the comprehensive daily support you need during this critical transition period. Our program can provide that support and is a worthwhile investment in your overall well-being.

Absolutely, your family remains central to your care, our services provide additional support to help ease their burden. We can also educate family members on medication management, warning signs to watch for, and care techniques so they feel confident supporting your recovery.

Yes. They can pick up most prescriptions with a few exclusions such as narcotics or benzodiazepines.

Our Rhythms Home Care RNs assess your needs and obtain physician orders for skilled, medical care that only CNAs can perform. This includes catheter care, oxygen management, compression therapy, and specialized transfer techniques based on your mobility requirements.

Yes, your care partner will come and help you pack, making sure nothing is left behind. Once you’re home, they’ll also help you unpack and organize everything exactly where you want it.

No contract required. You will sign service agreement forms to get started, and you can stop services anytime with 48 hours’ notice. Giving you complete flexibility as your needs change. For visit minimums: residents in our legacy communities have a 1-hour minimum per visit, while those in the greater Denver area have a 4-hour minimum per visit. There’s no weekly hour requirement, so you control the frequency and duration of care based on what works best for you.

Yes, you will be able to schedule as much time as you need for care. We can work with you for up to 24-hours.
