Medication Management at Rhythms Home Care

March 18, 2024

Whether you are settling into your sixties and seventies, or are someone who is caring for older loved ones, medication management is of utmost importance for aging adults. Despite our efforts to prevent it, our bodies change as we age which means that the way that medicine works in our bodies changes over time too.

What is Medication Management for Older Adults?

Aside from just the medications themselves, there are many things to consider when it comes to effective and efficient medication management.

These considerations include but are not limited to:

  • Accurate dosages and frequency of medication
  • Safe and proper storage methods
  • Various side effects and drug interactions
  • Input from various care providers
  • Schedule of refills
  • Cognitive impairment leading to forgetfulness

Since that list of considerations may be overwhelming, especially in addition to concerns of daily life, the team at Rhythms Home Care has introduced a new program. This service offering, Partnering for Your Success with Registered Nurse oversight, is designed to take on the mission of making medication more manageable for community members and their caregivers.

Taking these medications correctly is essential for healthy aging and overall symptom management. Therefore, putting a comprehensive system in place such as this new offering that takes care of the basics helps seniors avoid common mistakes in medication management.
