Meet the Team

About Mansour
With deep Colorado roots dating back to childhood, Mansour Gazawi serves as the Executive Director of Rhythms Home Care. After graduating from Regis University in 1998 with his Bachelor of Science in Nursing, Mansour began his professional journey at St. Anthony Central before transitioning to a 16-year career with the Broomfield Police Department as a Communications Specialist.
During his tenure with law enforcement, Mansour leveraged his medical expertise to educate dispatchers on handling emergency medical situations, receiving the Broomfield Police Employee of the Year award in 2016. His impact extended beyond the department through community education initiatives he developed on elder fraud prevention and recognizing medical emergencies—programs still in use today.
Following his 2019 retirement from law enforcement, Mansour entered senior care in 2022 at Brookdale Boulder, where he revitalized their home care program within a year. His success led to a promotion overseeing the Englewood and Denver Brookdale communities, where he achieved similar transformative results.
Mansour joined Christian Living Communities in May 2023, bringing his leadership and expertise to his current role as Executive Director of Rhythms Home Care. He approaches his work with a profound service-oriented philosophy, finding fulfillment in improving others’ lives. His problem-solving approach centers on active listening and collaborative solutions. When not working, Mansour cherishes time with his daughter and dog, considering family his highest priority.

About Lisa
With nearly 40 years of experience in healthcare, Lisa Case brings a wealth of knowledge and a deep passion for serving seniors to her role as the Business Development and Marketing Manager at Rhythms Home Care.
Lisa’s career began as a Certified Nursing Assistant (CNA), working hands-on with patients in intensive care, med-surg, orthopedics, home health, and hospice. Her experience also extends to managing the reuse department for a hemodialysis unit. These diverse roles have given her a comprehensive understanding of the healthcare landscape and a genuine appreciation for supporting seniors in their health journeys.
After relocating to Colorado in 2005, Lisa dedicated herself to senior care, including a decade with the Seniors Blue Book, where she strengthened her expertise in senior resources, networking, and marketing. Her extensive experience equips her to build meaningful connections, create impactful marketing strategies, and serve as a trusted resource for seniors and their families.
Outside of work, Lisa loves spending time with her family and traveling to Oklahoma to visit her two grandsons and other loved ones.

About Shaundra
As a Colorado Native having spent nearly 20 years caring for her local community, Shaundra Holmes joined the team in February 2024 as the new Rhythms Home Care Director of Nursing.
Shaundra’s nursing career began in a local Denver nursing home where her love for the senior community was found. She then transitioned to the neurology unit at Swedish Medical Center where she immersed herself in the world of hospital nursing.
Life soon took a significant turn for Shaundra that required her to shift her caretaking responsibilities from her patients to her family. Caring for her ailing father battling cancer, attending to her mother’s needs post-surgery, alongside embracing the joys of motherhood after her fourth child, prompted a prolonged leave from work. Despite the career break, the time away from the field granted her invaluable time with her loved ones as well as the perspective of what it takes to care for aging adults.
Shaundra jumped back into nursing right where she left off – in the neurology unit – later transitioning to the role of Employee Health RN at Swedish in 2013. From engaging new hires and coordinating employee flu vaccine clinics, to imparting essential education on hygiene and injury/infection prevention, Shaundra tackled each responsibility with compassion and empathy.
Shaundra’s experience and expertise led her to a new role of Health Services Coordinator at Colorado Christian University in December 2021.Though department downsizing eliminated Shaundra’s role at CCU, she firmly stands by the belief that, “when God closes one door, He opens another.” This open door led to the opportunity to serve the community of Rhythms Home Care.
In her role as Director of Nursing at Rhythms Home Care, Shaundra handles weekly medication management, a task she approaches with meticulous care. Through conducting thorough assessments, obtaining and managing medication orders, and organizing pill boxes and schedules, Shaundra is dedicated to the personalized, most optimal care for each client. Additionally, Shaundra gets to know each client on a deeper level by conducting regular assessments, coordinating medication refills, and providing essential medical services under doctor’s orders such as administering enemas, urinary catheter care, and management of stage 1 to stage 2 pressure wounds.
Beyond her clinical duties as the Rhythms Home Care Director of Nursing, Shaundra finds joy in educating care partners on a variety of topics such as infection prevention, dementia care, and customer service excellence. As a lifelong Colorado resident with a love for gardening, audiobooks, baking and family time with her children and grandchildren, Shaundra brings extensive professional expertise and a nurturing personal touch to her role as Rhythms Home Care Director of Nursing.

About Chase
Chase Halgerson joined Christian Living Communities in 2020, bringing a diverse skill set honed through roles at Holly Creek, Eagle Point Support Office, and Rhythms Home Care, where he currently serves as Senior Staffing Coordinator. During the 2020 Covid pandemic, he played a crucial role in procuring and supplying PPE, demonstrating his commitment to staff safety. He also personally assisted many other departments within CLC such as Donor Relations, Finance, and Sales & Marketing.
During his tenure with Rhythms, his expertise extends to scheduling building, customer service, billing & financial reporting, and administrative support. Prior to CLC, Chase spent five years in food and beverage procurement at Telluride Ski Resort and Levy Restaurants at CU Boulder, building a strong foundation in customer service and team collaboration. He holds a Bachelor of Science in Economics from the University of Utah.
Outside of work, Chase enjoys spending time with his family, exploring nature, visiting breweries, and cheering on his favorite sports teams.

Rhythms Home Care is a service of Christian Living Communities (CLC) a non-profit organization enriching the lives of older adults since 1972. Rhythms Home Care was established in 1995 as a division of CLC, and continues to build on over 25 years of trusted service in Colorado.
As a non-profit we are accountable to an all-volunteer board of directors rather than shareholders. Rhythms Home Care prioritizes client and team member well-being above all else, and is not required to maximize profits in the interest of shareholders as for-profit companies are.
